Contribution Remittance Forms

For your convenience we have developed this Connecticut Conference Remittance Form, and made it available in three formats (Microsoft Word, Microsoft Excel, and Adobe PDF). The form will help clarify your intent when sending financial contributions to the Connecticut Conference.

Please make copies of this form and use it when you are sending in donations for Our Church’s Wider Mission, (OCWM) including Basic Support, One Great Hour of Sharing, Neighbors in Need, Strengthen the Church, and the various directed gifts that flow through to the National UCC.

The remittance form also includes a space to provide Connecticut Conference Per Capita contributions, as well as donations to Friends of the CT Conference, Silver Lake Conference Center, Silver Lake Scholarships, Now for the Future, Youth and Young Adult Ministry, The Colombian Partnership and the Korean Partnership.
